今日の検査・診察は、
・リハビリ課の診察
・CT（造影剤なし）
・麻酔科の診察
・主治医の先生の手術前の説明
自分の場合、腫瘍が言語中枢の近く、具体的に言うと言語を理解する部分と発語するための部分をつなぐ回路のすぐそばにある。これはfMRI（Functional MRI）という検査をするとわかる。4月の検査入院の時にfMRIを受たが、画像をみたら言語はオレンジ、運動はムラサキという感じで結構はっきり写っててスゲーっ！って思った。
腫瘍を取る時にこの回路に影響を与えると、話すことがしにくくなるのでリハビリでそれを補う方法を身に着ける。手術後は言語のリハビリをメインで、運動に関する部分に影響する可能性は低いので、何も無ければ運動のリハビリはしないとのこと。
あと、主治医の先生から手術前の説明があり、家族と一緒に受けた。検査入院時や外来で聞いてた話も含めて説明があった。要旨は以下、
・診断は神経膠腫のグレード2の可能性が高い。確定は生検後（取った腫瘍細胞を調べる）。
・無症状の場合の標準的な治療指針は無いが、最近の研究では取ったほうが良いことが分かっている。（早く取ってほしいとこちらからも希望した）
・悪性腫瘍なのでできるだけ取る。が、言語への影響が出るならそこで止める。
・言語能力への影響を最小限にするため覚醒下で手術する。手術中に麻酔を覚まして言語タスクを行う。脳に電気刺激を与えながら言語にかかわる部分を麻痺させて、しゃべれるかどうか確認することで言語にかかわる部分を特定する。
・電気刺激が正常に与えられているかどうかを確認するため、運動中枢も同様に刺激する。運動中枢は人による位置の違いがあまりないので確認のためやる。
・上記の確認とどこから腫瘍にアプローチするかはやってみない部分があるので、頭骨を広い範囲で開ける。
・ナビゲーションシステムを使って術部の位置を確認しながらやる。カーナビみたいなもの。
・手術時間はセットアップに2h、手術は6hでうち覚醒下は2hくらい。
・術後CTを撮ってICUへ一晩入る。何もなければ翌朝には一般病棟に戻れる。
あと、この手術で考えられるリスクは、
・言葉をしゃべる機能に影響する可能性が高い。書くのは大丈夫っぽい。
・血管を傷つけると患部から離れたところ（血管の下流側）の脳がやられる。腫瘍のグレードが高いと血管が多い。
・けいれんの可能性がある。
・など、
あと、いろいろな事柄に対して同意を求められるので、あとで同意書にサインして提出する。もちろんオールOK。
あと、他の研究機関と合同でグリオーマの遺伝子研究をやっていて、遺伝子検査してもよいか確認があった。遺伝子検査しておくと再発時にも治療の方針が立てやすいとのこと。この検査を自費でやると保険がきかずとてつもなく高額になるらしいが研究費で出してくれるらしい。医学の発展と自分のためにももちろん同意オーケー。
可能性の高いものから低いものまでいろいろなリスクを説明してくれたけど、先生の説明を受けた印象では、まあ大丈夫だろうと思った。
びまん性星細胞腫グレード2を患ってます。 主観的辛痛度:★☆☆☆☆（医学生の教育への対応も求められたがもちろんオッケー。自分についたのは女子学生。病室へ話に来てくれてとても和む。）

My tumor is near by speech center which my tumor get into a circuit between a part of understanding of the language and a part of speech. If you are examined by fMRI(Functional MRI), you can image in your head. I was hospitalized for examination on April and examined by fMRI, I am surprised with clear images that parts of language were orange and purple were parts of motion.
I might have some difficulties to speech things, if the circuit is damaged to take out the tumor. Therefore I will mainly take some rehabilitation to get how to support. However I do not motion rehabilitation because I do not have any problem at the surgery.
My doctor explained about the surgery, I and my families leaned from it. Parts of the explanation were already explained at exam. hospitalized or outpatient care. summary is,
* The tumor might be the grade 2 of gliomas. Final decision will be after a biopsy.
* There is not a normal guideline to treat it. But some paper on some medical society said that taking out of the tumor will be good for prognosis. (I hoped I took the surgery)
* The grade 2 is a malignant tumor so that it will be take out as much as possible. However if I have a difficulty of speech center, that are not touched.
* The surgery will be operated in an awake surgery to minimize the effect for my speech search. The awake surgery is to be wake up and task things and identified an location of speech center to confirm whether I talk or not with an electrical impulse.
* An motion center will be impulsed to be confirmed whether the impulses are correct normally. The motion center is as same as almost humans.
* An opening are of my skull is opened wider because it is mentioned above and it is confirmed a corse to the tumor.
* An operational position in my head is located by a navigation system like a car navigation.
* It takes 2 hours for set-up and 6 hours for the surgery in 2 hours.
* After the surgery, I will have CT and go to ICU. I will go to my ward next morning.
And expected risks are,
* An effected to talk things will be effected. Writing will be no problem.
* If a blood vessel is damaged, brains downstream from it are damaged. The high grade tumor has many blood vessels.
* A convulsion.
* Etc.
And I signed all documents for many things. Of course they are OK.
My doctor said that his hospital is researching a gene of glioma with other research center. Are they OK that your gene be investigated for our research and for me to consider the course of my treatment. This gene inspection fee costs pretty high if I pay, but the fee costs on the research budget. I answer OK for medical science and me.
My doctor talked us about many risks including high risk and low risk. I felt quite safe.
Subjective difficulty rate: ★☆☆☆☆(Medical student will learn from me. It is OK. She often come to my room to talk with me. She gives me cheerful mind.)
